Lifeline Call Plan Requirements

Valid Photo ID (Can be driver's license or state-issued ID) .

Proof of current benefits ( Oklahoma or Federal-based). Medicaid and Social Security Disability (SSI) are accepted, however, Medicare and retirement Social Security are not acceptable forms of benefit.

A variety of phones are available to lifeline customers. You may qualify for a free phone ( the model will vary), or select a different model for an additional cost. Ask our Pine representatives to explain your options.

Services such as text messaging and data ( internet) are not included. These services are available for an additional charge.
